Office Hours & Appointments
Our office is open from 8:30am to 5:00pm Monday through Friday, with the exception of major holidays. Appointments can be made by calling (360) 697-6547. If you are not able to keep your appointment, please call at least 24 hours in advance to reschedule, so that we may offer your appointment to another patient.

Financial Policies
We participate in most insurance plans, and file insurance claims on patients’ behalf for office visits and surgeries. Patients are responsible for obtaining any referrals required by their insurance carrier. We expect patients to make co-payments, and/or deductible payments, at the time services are rendered. If you have questions regarding this policy, please contact our Billing Department prior to your appointment.
